The only options I have, through the phones ‘Display’ link is to choose from Gallery and Wallpaper, neither of which contain the ICS Theme.
Phone is rooted, so is there a way to add this app to my lockscreen?

If so, the launcher can’t find it when the phone is starting and that’s why you have to re-apply each time. Move the theme back to the phone and you won’t have this problem.
